Registering individual students for service

To qualify for service, an individual student must have a visual or physical limitation or an organic reading disability (such as dyslexia) that prevents them from using standard print. Please complete a yellow individual application (form may be downloaded from this site) to register a student for school and/or home service. Please follow all the guidelines as listed on the application.

  • Registered students may borrow one audio player for use at school and/or may have a separate account and player at home.
     
  • The teacher is responsible for all audio players and materials on loan directly to his or her students for use at school.
     
  • The student's family is responsible for any audio player and materials on loan to him or her for use at home.
     
  • All audio players on loan to an individual student at school must follow that student to his or her classroom each year, or be returned to your local Talking Book Center. We will contact teachers each school year, asking for information to facilitate this process.
     
  • An audio player on loan to a student is intended for use by that specific student and is not to be used with students who are not print-impaired.
     
  • To remain registered with the program a student must actively borrow materials from the Talking Book Center. If no materials are borrowed, the audio player on loan to the student must be returned and service will be canceled.
